INCENTIVE FORM

Preventative Services Guidelines for 2012 Incentive Program

**Incentive gift cards will be distributed August and December 2012, and March and May 2013. To be included in a distribution, incentive forms must be returned to Wellmark by the 15th of the prior month (July 15, November 15, February 15 and April 15).

**Gift cards are considered taxable income and will be reported to the IRS at year-end via Form 1099-Misc.
